It's whatever sorts of antisocial behavior are not explicitly allowed in
your game. EG, if your game is not about PvP, then PvPers are in that group.
If PvP is permissible, then they are acceptable players.

Each one of those can cost you ten players or more who quit over what was
done to them.

It works out at quite less than 5%. What he (we) use is a historical log of
incidents for every reported incident, and that we can can judge when
someone has crossed a line. Some sorts of offenses are "first strike" and
some are "three strikes"...
